Elke maand organiseert ECEZG een Journal Club Fysiotherapie in Groningen. Maandelijks staan er vaste onderdelen op het programma, waarvan op deze blog achteraf een samenvatting komt te staan. Het hoofdonderwerp van de Journal Club van 2 februari was het effect van (Transcutane) Electro Neuro Stimulatie op spierfunctie bij patiënten op de Intensive Care Unit.
Praktijkgerichte vraag
‘Wat is het effect van (Transcutane) Electro Neuro Stimulatie op spierfunctie bij patiënten op de Intensive Care Unit?’
Bedlegerige patiënten op de IC verliezen langzaam hun spierfunctie. Gezien deze groep niet in staat is om oefentherapie te volgen, zou het wellicht een optie kunnen zijn om (Transcutane) Electro Neuro Stimulatie (TENS) toe te passen in de praktijk. De vraag vanuit de Journal Club is echter, is het effect hiervan al reeds bekend of zijn er aanwijzingen dat dit een toevoeging aan de behandeling kan zijn?
NeuroMusculaire Electro Stimulatie (NMES) bij ICU patiënten

De afgewogen formulering van de gevonden artikelen over NMES bij ICU patiënten luid als volgt:
Er zijn de afgelopen jaren redelijk wat publicaties geweest over de toepassing van NMES bij ICU patiënten. Allereerst is het belangrijk te definiëren welke mogelijke oorzaak de spierzwakte heeft. Er zijn vier mogelijk oorzaken: malnutritie, cachexie, sarcopenie en trauma/sepsis. Indien er een sarcopenie optreedt kan de reden van spierzwakte door twee oorzaken komen: Sarcopenie (type II vezels) met of zonder denervatie van de type II vezels.
De meeste populaties die geïncludeerd zijn beschrijven veel patiënten. In vele gevallen is er maar een klein aantal patiënten geïncludeerd in de studie. Dit kan komen door de inclusieprocedure en/of de geschiktheid van de therapievorm op deze patiënten categorie.
Vanuit een analyse uit de Leuven groep van onderzoekers zijn er geen belemmeringen om NMES toe te passen. Vanuit het praktisch oogpunt is het bijzonder gecompliceerd om de juiste patiëntencategorieën te vinden om te behandelen. Therapeutische criteria van behandelduur, intensiteit en lokalisatie worden beperkt beschreven. De effecten op spierdikte en krachtsbehoud zijn positief te noemen.
Vanuit eigen ervaring, zie ik liever dat de mate van sedatie overdag gereduceerd wordt en dat er actief geoefend kan worden. ‘s Avonds kan de sedatie weer opgehoogd worden indien nodig bij bv. onrust. Dit heeft voor het algeheel functioneren van de patiënt veel meer positieve effecten dan het gebruik van NMES gedurende de dag.

Resultaten
Drie knijpkrachtmeters werden onderzocht op zowel betrouwbaarheid als op validiteit. Daarnaast werden ze in verschillende houdingen getest, om de invloed hiervan te bekijken. De Jamar, DynEx en TKK knijpkrachtmeters werden getest; waarbij er een systematische meetfout van -1,92, -1.43 en 0.49 kg gevonden werd voor de knijpkrachtmeters.
Conclusie
Hoewel met alle drie de meters de knijpkracht op een zeer degelijke wijze gemeten kan worden, blijkt de TKK dynamometer de hoogste diagnostische accuratesse te hebben.
Het verschil in ellebooghoek werd alleen gevonden met de TKK dynamometer. De reden hiervan is niet gevonden binnen het artikel. De deelnemers van de Journal Club kwamen tot de hypothese dat wellicht het comfort tijdens de meting invloed heeft; indien je niet harder kan knijpen omdat het gevoelig wordt tijdens het knijpen zal je mogelijk een plafond krijgen. Mogelijk is het comfort bij de TKK hoger waardoor deze wel méér kracht registreert tijdens een geflecteerde elleboog.

Providing manual therapy (mobilisations of knee and hip, manipulation of hip, ankle manipulation and/or mobilization, LWK manipulation) in addition to exercise therapy improved treatment effectiveness compared to providing 12 consecutive exercise therapy sessions.

Nieuwe Praktijkgerichte Vraag
Tijdens elke Journal Club wordt er gezamenlijk een praktijkgerichte vraag geformuleerd waar alle deelnemers een antwoord op proberen te vinden door een zoektocht naar artikelen te starten.
De vraag voor de volgende keer luidt: ‘Wat is het effect van mobilisaties op pijn bij patiënten met musculoskeletale problemen?’
Een basistechniek die door vele fysiotherapeuten gebruikt wordt; bij pijn gaat men mobiliseren dan wel oscilerende tracties toepassen. In welke mate zijn deze interventies effectief voor pijndemping?
